Efficacy of adalimumab in moderate to severe hidradenitis suppurativa: Real life data | Dermatology Reports

Abstract: Hidradenitis suppurativa (HS) is a relapsing, inflammatory disease characterized by painful nodules, abscesses, sinuses track formation and scarring. HS has a great impact on patients’ quality of life and its treatment may be really challenging. Adalimumab provides a new therapeutic option for HS. Our aim was to assess the therapeutic potential of adalimumab on patients with HS based on the data from the daily clinical practice of an HS Outpatient Clinic. 19 patients with clinically evident moderate to severe HS, under adalimumab treatment for at least 24 week, participated in this observational, retrospective study. The Hidradenitis Suppurativa Physician’s Global Assessment scale, Modified Santorius scale and Dermatology Life Quality Index (DLQI) at baseline, week 4, week 12 and week 24 were retrieved from the records. Both Modified Santorius score and DLQI were significantly decreased during the weeks of evaluation (Friedman’s test; P < 0.001). The proportion of patients who achieved clinical response was 10.5% (n = 2) at week 4, 42.1% (n = 8) at week 12 and 63.2% (n = 12) at week 24. Treatment with adalimumab was linked with both clinical remission of HS and improvement of patients’ quality of life

Abstract: 39 bullous pemphigoid (BP) patients were studied to assess the clinical significance of anti-BP180 and anti-BP230 circulating autoantibodies of BP and correlate their titers with the clinical scores of the BP Disease Area Index (BPDAI) and the Autoimmune Bullous Skin Disorder Intensity Score (ABSIS) as well as with the intensity of pruritus measured by the BPDAI pruritus component. All parameters were evaluated by the time of diagnosis (baseline), month 3, and month 6. Titers of anti-BP180 autoantibodies were strongly correlated with BPDAI (, ) and ABSIS (, ) values, as well as with BPDAI component for the intensity of pruritus (, ) at baseline. At month 3, titers of anti-BP180 autoantibodies were strongly correlated with BPDAI (, ) and ABSIS (, ) values, as well as with the BPDAI component for the intensity of pruritus (, ). At month 6, titers of anti-BP180 autoantibodies were strongly correlated with BPDAI (, ) and ABSIS (, ) values, as well as with the BPDAI component for the intensity of pruritus (, ). Introduction Bullous pemphigoid (BP) is the most common subepidermal autoimmune bullous disease. Detection of anti-BP180 and/or anti-BP230 serum autoantibodies by commercially available ELISA kits has almost become nowadays a routine method for the diagnosis and followup of BP patients [1–3]. During the last decade, efforts to evaluate the clinical extent and severity of autoimmune bullous diseases, generally, have led to the establishment of scoring systems. Up to today there are two already validated scoring systems for pemphigus, the Autoimmune Bullous Skin Disorder Intensity Score (ABSIS) and the Pemphigus Disease Area Index (PDAI) [4]. ABSIS has been introduced in 2007, in order to achieve an improved evaluation and monitoring of the status of both oral and cutaneous lesions in patients with pemphigus. It has been used, though, in other autoimmune bullous diseases, such as BP and epidermolysis bullosa acquisita [5]. The Pemphigus Disease Area Index (PDAI) was developed by the International Pemphigus Definitions Committee in 2008, to evaluate both mucosal and cutaneous lesions [6]. In 2011, the International Pemphigoid Committee proposed the BP Disease Area Index (BPDAI), in order to achieve a consistent reporting of the outcomes in BP reports and studies [7]. Abstract: One of the fundamental principles of statistical physics is that only partial information about a system's state is required for its macroscopic description. This is not only true for thermal ensembles, but also for the unconventional ensemble, known as Generalised Gibbs Ensemble (GGE), that is expected to describe the relaxation of integrable systems after a quantum quench. By analytically studying the quench dynamics in a prototypical one-dimensional critical model, the massless free bosonic field theory, we find evidence of a novel type of equilibration characterised by the preservation of an enormous amount of memory of the initial state that is accessible by local measurements. In particular, we show that the equilibration retains memory of non-Gaussian initial correlations, in contrast to the case of massive free evolution which erases all such memory. The GGE, being a Gaussian ensemble, fails to predict correctly the equilibrium values of local observables, unless the initial state is Gaussian itself. Our findings suggest that the equilibration of a broad class of quenches whose evolution is described by Luttinger liquid theory starting from a non-Gaussian initial state, is not correctly captured by a GGE, raising doubts about its validity in general one-dimensional gapless integrable systems such as the Lieb-Liniger model. We also propose that the same experiment by which the GGE was recently observed [Langen et al., Science 348 (2015) 207-211] can also be used to observe its failure, simply by starting from a non-Gaussian initial state.
Experiential dimensions and their influence on behavioral intentions within the context of nature-based tourism

Abstract: Sa?etak Purpose – This paper aims at investigating the customers’ experience in nature-based attractions and its influence on post consumption behavior. Design – The study uses two theoretical frameworks (experience economy and behavioural intentions) to investigate the determining dimensions of the wildlife experience to customer satisfaction, and the relationships between the wildlife experience and the behavioural intentions. Approach – The research develops an explanatory framework of consumer experience that measures the four dimensions and their relative influence on satisfaction, perceived service quality and behavioral intentions. Methodology – The proposed research framework was empirically tested by implying a quantitative research and the technique of convenience sampling to customers of Game Reserves in Republic of South Africa. Findings – The results indicate that the experience framework constitutes a valid tool for the study of wildlife experiences. In the context of nature-based tourism the experiential dimensions which influence the perceived service quality and customers’ satisfaction are escapism and esthetics. There is also a strong correlation between wildlife experience and post consumption behavioural intentions, especially for word-of-mouth and repurchase intention. Originality – The research extends existing theory by incorporating new elements and empirically investigating them within a new context. It is the first empirical study that investigates the experiential dimensions within the context of nature-based tourism and customers’ behavioural intentions. It develops an explanatory framework of customer experience that measures the four dimensions, their relationship with experience outcomes (satisfaction and perceived service quality) and their relative influence on behavioural intentions
Quantum quench in interacting field theory: a self-consistent approximation

Abstract: We study a composite quantum quench of the energy gap and the interactions in the interacting \phi^4 model using a self-consistent approximation. Firstly we review the results for free theories where a quantum quench of the energy gap or mass leads for long times to stationary behaviour with thermal characteristics. An exception to this rule is the 2d case with zero mass after the quench. In the composite quench however we find that the effect of the interactions in our approximation is simply to effectively change the value of the mass. This means on the one hand that the interacting model also exhibits the same stationary behaviour and on the other hand that this is now true even for the massless 2d case.
Quench echo and work statistics in integrable quantum field theories

Abstract: We propose a boundary thermodynamic Bethe ansatz calculation technique to obtain the Loschmidt echo and the statistics of the work done when a global quantum quench is performed on an integrable quantum field theory. We derive an analytic expression for the lowest edge of the probability density function and find that it exhibits universal features, in the sense that its scaling form depends only on the statistics of excitations. We perform numerical calculations on the sinh-Gordon model, a deformation of the free boson theory, and we obtain that by turning on the interaction the density function develops fermionic properties. The calculations are facilitated by a previously unnoticed property of the thermodynamic Bethe ansatz construction.
